Self-caused suffering and the approach of death The sufferers are called “fools,” not mainly as an insult but as a description of people who act destructively. Their affliction is linked to their disobedience and “iniquities,” and the illness is so severe that they cannot stand food. The image “gates of death” portrays them as arriving at the threshold of dying.
Crying out and being healed by a sent word At the crisis point they cry to Yahweh in their trouble. Yahweh responds by bringing them out of their distresses. The rescue is pictured as Yahweh sending his word, which results in healing and in being “delivered” from their graves—language that emphasizes reversal from imminent death.
The fitting public response The psalm turns from narration to exhortation: these rescued people should praise Yahweh for his loyal love and remarkable deeds toward humans. They are urged to offer “sacrifices of thanksgiving” and to tell about his works with singing, making gratitude visible and audible in the community.
Literary Context
Psalm 107 is a series of scenes that repeat the same movement: people get into deep trouble, they cry to Yahweh, he delivers them, and they are urged to thank him. The “sick” scene in vv. 17–22 is one of several snapshots (alongside travelers, prisoners, and storm-tossed sailors) that together portray many kinds of distress. The repeated call to praise highlights the psalm’s aim: to turn rescue stories into community worship. This unit also introduces the striking line about Yahweh healing by “sending his word,” which summarizes the turning point from near-death to restoration.
Historical Context
The psalm speaks in broad, teachable pictures rather than naming a single event. It assumes a world where serious illness could quickly become life-threatening, where loss of appetite was recognized as a sign of extreme danger, and where death could be imagined as entering city gates. It also assumes an Israelite worship setting in which people respond to deliverance with thanksgiving offerings and public proclamation through song. The language fits a community that tells and retells such rescues as shared memory, instructing later hearers how to interpret crisis and recovery in everyday life.
Theological Significance
These verses present a repeating pattern found across Psalm 107: people end up in extreme distress, they cry out to Yahweh, and he rescues them. Here the distress is severe illness that is connected to their own destructive wrongdoing (v. 17) and pictured as being near death (v. 18). The turning point is Yahweh’s response: he saves them, “sends his ,” heals them, and pulls them back from the brink of the grave (vv. 19–20). The fitting outcome is public gratitude expressed in worship and testimony (vv. 21–22).

The passage treats Yahweh as able to reverse a life-threatening condition by effective speech. It also treats thanksgiving as something that belongs in community life, not only in private feeling.
One question is how direct the link is between sin and sickness in this scene. Some read the passage as describing a general moral cause: certain suffering is the consequence of stubborn wrongdoing, and this case is an example. Others read it more narrowly: the psalm is describing one kind of suffering story (self-caused), without claiming that sickness in general should be traced to personal wrongdoing.
Another question is what “sending his word” means. Some take “word” mainly as a divine command that accomplishes healing. Others see it as a promise or message of rescue, possibly carried through an agent or ordinary means, with the focus on Yahweh as the true source of the outcome.
A third question is how literal “their graves” is (v. 20). Some take it as near-death language (they were almost dead). Others allow that it could include actual deliverance from death, expressed with poetic compression.
The psalm uses vivid, compressed poetry rather than a case report. It links wrongdoing and affliction in v. 17 but does not pause to state how broadly that link should be applied. Likewise, “word” is potent but flexible language: it can mean command, promise, or message, and the verse does not specify the mechanics.
Explicitly, it claims that (1) some people’s severe suffering can be tied to their own disobedience, (2) the crisis can become so intense it looks like the doorway to death, (3) crying to Yahweh is the narrated turning point, and (4) Yahweh’s rescue is described as healing that comes through his “sent word,” pulling them back from the grave (vv. 17–20). By inference, the passage supports a theology where Yahweh’s speech is not merely informative but active and effective, and where rescue rightly becomes shared praise and publicly told story (vv. 21–22; compare Psalm 107:1 for the repeated theme of loyal love).
